St. Mary's Phlebotomy Program Western Slope School of Phlebotomy Enrollment begins: November 10, 2011 Application deadline: December 27, 2011 Program duration: January 21, 2012 through May 12, 2012

Classes will be held for twelve Saturdays beginning January 21 through April 21

Internship includes 6 rotations lasting 6 weeks, 20 hrs per week for 120 hours total

Program tuition and fees: $1400.00

If you are unable to download these forms, they will be available at Laboratory Services in the hospital (Main Floor, River Zone).
